Overnight Program Request for Students 5th Grade through College

School Year Program Request Form is for schools or group of 20 or more participants in need of a residential/overnight program. This form is required to be considered for available dates with SEACAMP San Diego. We are currently scheduling for Fall 2024. We are not yet booking for Spring 2025, if you are interested in attending during this time please complete this form and we will be in contact soon.

Per the California Regulation all participants (students and adults) are required to provide a copy of their immunization records that include the date of their received last tetanus shot.
